? Steps to Schedule a Telehealth Appointment

  1. Go to the HLTHI.life Patient Website
    Visit https://patients.hlthi.life/welcome

  2. Click on “Schedule a Visit”
    On the main screen, select the “Schedule a Visit” button to begin.

  3. Select Your Location
    Choose the state or region where you’re seeking care.

  4. Select the Patient
    If multiple patients are linked to your account (such as children or dependents), select the appropriate one.

  5. Confirm Date of Birth
    Enter the patient’s date of birth to verify identity.

  6. Fill Out Patient Details
    Provide the necessary information, including:

    • Email address

    • Reason for visit (e.g., cold symptoms, prescription refill, follow-up, etc.)

  7. Confirm Appointment Information
    Review all entered details and appointment time. If everything is correct, finalize the booking.


? After Booking

  • A confirmation message will appear on screen

  • You’ll receive an email with the appointment details

  • The appointment will be visible under your “Appointments” tab when logged in
