Signing and Managing Your Provider Agreement

Your Provider Agreement outlines your relationship with HLTHi and must be signed before you begin seeing patients. You can view and download your contract at any time from the Contracts tab.

Accessing Contract Management

  1. Log in to the provider portal.
  2. Click Contracts in the left sidebar.

You will see a list of contracts with status indicators, such as:

  • Signed
  • Unsigned
  • Pending

Signing Your Provider Agreement

  1. On the Contracts page, locate the Provider Agreement.
  2. If unsigned, click View to open the contract.
  3. Follow the prompts to review and electronically sign the agreement.
  4. Once completed, the status will update to Signed, and the signed date will be displayed.

Viewing or Downloading Your Contract

  • Click View to open the agreement in the browser.
  • Click Download to save a copy of the signed contract.

What If My Contract Is Missing?

If you do not see a contract listed or if the status looks incorrect, please contact support with:

  • Your full name
  • The email address associated with your HLTHi account
  • A brief description of the issue
