Completing Your Provider Application & Profile

Before you can start seeing patients on HLTHi, your provider application and profile must be fully completed and approved. This article explains each step and how to reach 100% completion.

Application Status

At the top of your dashboard you will see an Application Status banner:

  • Pending – We are reviewing your information.
  • Approved – You are cleared to practice on the platform (subject to active license and subscription).
  • Rejected / Needs Update – Additional information or corrections are required.

The Profile Completion bar shows how many required steps you have finished. You must complete all required steps (licenses, documents, contact information, credentials, payout setup, and subscription) for full activation.

Steps to Complete Your Profile

  1. Add at least one state medical license in the Licenses tab.
  2. Sign your Provider Agreement in the Contracts tab.
  3. Upload required documents (medical degree and driver’s license) in the Documents tab.
  4. Fill out your contact information (email, phone, address) in the Contact Info tab.
  5. Enter your NPI and tax identifiers in the Credentials tab.
  6. Upload driver’s license details in the Driver License tab.
  7. Complete Stripe payout setup in the Payouts tab.
  8. Activate your subscription in the Subscription tab.

When Will My Application Be Approved?

Once all required steps are completed, our team will review your application. You will receive an email when your application is approved or if additional information is needed.

If your status stays “Pending” longer than expected, please submit a support ticket with:

  • Your full name
  • The email address on your HLTHi account
  • Any recent updates (new license uploaded, contract signed, etc.)
