Uploading Required Documents
The Documents page is where you upload your required credentials for onboarding, such as your medical degree and driver’s license. These documents are used for verification and compliance.
Accessing Document Management
- Log in to the provider portal.
- Click Documents in the left sidebar.
You will see a banner at the top indicating your Document Completion status and a list of required and optional documents.
Required Documents
Typical required documents include:
- Driver's License
- Medical Degree
Each card displays:
- Document type and internal label (e.g., driver_license, medical_degree).
- Status: Uploaded or Missing.
- The upload date.
How to Upload a Document
- On the Documents page, click the document card that needs to be uploaded.
- Use the file picker to select a PDF or image (JPG/PNG) from your device.
- Confirm the upload and wait for the success message.
Once all required documents are uploaded, the banner will show that your documentation is complete.
Viewing or Downloading Documents
For any uploaded document:
- Click View to preview it in the browser.
- Click Download to save a copy to your device.
Optional Documents
If optional documents are supported, they will appear in the Optional Documents section. While not required for onboarding, they can help speed up future credentialing.
If a Document Is Rejected
If a document is invalid or unreadable, our team may request that you upload a new copy. Common reasons include:
- Blurry or cropped images
- Missing pages or seals
- Upload of the wrong document type
If you are unsure why a document was rejected, please reach out to support with the document name and upload date.
