Driver's License & Identity Verification

As part of compliance and credentialing, HLTHi requires a valid government-issued ID, such as a driver's license. This is used solely for identity verification and is stored securely.

Accessing the Driver's License Page

  1. Log in to the provider portal.
  2. Click Driver License in the left sidebar.

You will see:

  • Issuing State
  • License Number
  • License Image status (e.g., "No image uploaded").

Adding or Updating Your Driver’s License Information

  1. Click Edit in the upper-right of the License Information card.
  2. Enter your issuing state and license number.
  3. Upload a clear image of your driver’s license (front side at minimum).
  4. Save your changes.

Identity Verification Notice

The Identity Verification section explains why this is required:

  • Used for identity verification as part of your application.
  • Stored securely and only shared with authorized personnel.
  • Required for compliance with healthcare provider credentialing standards.

Fixing “Image Load Error: Invalid File Detected”

If you receive an Image Load Error message:

  • Make sure your file is in a supported format: JPG, PNG, or PDF.
  • Verify the file is not password protected or corrupted.
  • Ensure the file size is within the allowed upload limits.
  • Try re-saving the image or taking a new photo.

If the error continues, contact support and include:

  • The file type you attempted to upload.
  • A brief description of what you see on screen.
  • A screenshot of the error if possible.
