If you no longer wish to use HLTHI.life, you can request permanent deletion of your account and personal data. Here's how.

Important Before You Delete

Deleting your account is permanent. Once deleted:

• You will lose access to all appointment history and health records

• Any linked family members or dependents will also lose access

• This action cannot be undone

How to Request Account Deletion

1. Send an Email to Our Support Team
   Email us at patients@hlthi.life to request deletion.

2. Include the Following Information:

   • Your full name

   • The email associated with your HLTHI.life account

   • A simple statement confirming your request (e.g., “I request permanent account deletion”)

3. Identity Verification
   Our team may follow up to verify your identity before processing the deletion.

4. Confirmation
   Once processed, you’ll receive a confirmation email. Your data will be permanently deleted from our system in compliance with HIPAA and data privacy laws.
