? Steps to Register

1. Go to the Patient Welcome Page
   Visit: https://patients.hlthi.life/welcome

2. Click the "Register" Button
   On the welcome page, click "Register" to begin the sign-up process.

3. Enter Your Personal Information
   Fill in the required fields, such as:

   • First name (e.g., Joe)

   • Middle name (optional)

   • Last name (e.g., Doe)

   • Date of birth (e.g., 03/16/2009)

   • Birth sex (e.g., Male)

   • Email address (e.g., joe.doe@example.com)

   • Phone number (e.g., (555) 123-4567)

4. Agree to Terms & Conditions
   Check the box to confirm you agree to the Terms of Service and Privacy Policy.

5. Verify Your Email
   After submitting your registration, check your email inbox for a verification link.
   Click the link to activate your account.
   Note: If you don’t see the email, check your spam or junk folder.
