How to View Your Appointment History

Need to see past or upcoming visits? Here’s how to access your full appointment history on HLTHI.life.

Steps to View Your Appointment History

1. Log In to Your HLTHI.life Account
   Visit https://patients.hlthi.life/welcome and sign in with your email and password.

2. Click on “Past Visits”
   Once logged in, select the “Past Visits” tab from your dashboard menu.

3. Select the Patient Profile
   If your account has multiple family members or dependents, choose the patient whose appointment history you want to view.

4. View Upcoming and Past Appointments

   • Upcoming visits will appear first, listed by date

   • Scroll down to view Past Visits, which show completed or missed appointments

5. Click a Visit to See Details
   For past visits, click to view the provider’s name, date and time, reason for visit (if entered), and any available notes.

• Appointment history is organized chronologically

• If you’ve scheduled visits for family members or dependents, make sure you’ve selected the correct profile
